YOU ARE HEREBY SUMMONED to answer the complaint in this action and to serve a copy of your answer, or, if the complaint is not served with this summons, to serve a notice of appearance on the plaintiffs attorneys within twenty (20) days after the service of this summons, exclusive of the day of service, or if the service of this summons is made by any means other than by personal delivery to you within the state, within thirty (30) days after such service is complete. In case of your failure to appear or answer, judgment will be taken against you by default for the relief demanded herein.

PLEASE TAKE NOTICE that the nature of this action is Absolute Divorce. The relief sought is: see Complaint.

“Statement of Nature of Action”
The relief sought is a judgment of absolute divorce in favor of the Plaintiff dissolving the marriage between the parties in this action. The nature of any ancillary relief demanded is as follows: a) That neither party shall be obligated to pay maintenance to the other party following the dissolution of the parties' marriage; b) That the marital property shall be equitably divided in the following manner: The Plaintiff shall be awarded all the marital property currently in her possession, and the Defendant shall be awarded all the marital property currently in his possession ; c) That the Plaintiff may resume use of her premarital surname, Myrick, or any former surname; and d) That the Court grant such other and further relief as the Court may deem just and proper
