FIRST INSERTION
CLAIM OF LIEN
Before me, the undersigned authority, personally appeared EMILY L. LANG, ESQ. who, being duly sworn, states that:
1. I am an attorney with Lang & Brown, P.A., the legal representative and authorized agent of CHEVAL PROPERTY OWNERS' ASSOCIATION, INC. (the “Lienor”), whose mailing address is P.O. Box 7990, St. Petersburg, Florida 33734.
2. The Master Declaration of Covenants, Conditions and Restrictions for Cheval Polo & golf Club (The “Master Declaration”) authorizes the Lienor to levy assessments against Cheval lots, units and patio homes and creates a continuing lienor in favor of the Lienor for such assessments, together with interest thereon as provided below, costs and reasonable attorneys' fees. The Master Declaration specifies that any such assessment not paid within thirty (30) days after the due date shall bear interest from the due date until paid at the rate of eighteen percent (185) per annum.
3. LOCATIONS, INC. is the record owner of a certain lot, unit or patio home described as follows:
Lot 1, Block 4, CHEVAL POLO AND GOLF CLUB PHASE ONE-A, according to the plat thereof as recorded in Plat Book 57, page 59, of the Public Records of Hillsborough County, Florida.
4. There have been duly made by the Lienor upon said lot, unit or patio home the following quarterly assessments, which are due, payable and unpaid, and for which Lienor claims a lien on said lot, unit or patio home:
