Hurst (formerly known as Nellie Ruth Hurst and Nellie Ruth Lane Crunk Hurst), deceased, File Number 2011-CP-1978 by the Circuit Court for Manatee County, Florida, Probate Division, the address of which is P.O. Box 25400, Bradenton, FL 34206; that the decedent's date of death was July 15, 2011; that the total value of the estate is $93,600 less exempt assets and that the names and addresses of those to whom it has been assigned by such order are:
