Please take notice that Source Documents & Information, Inc., a Florida corporation, was dissolved on May 18, 2011. Any person having a claim against the corporation must send a notice of claim to: Scott P. Swope, J.D., Swope & Bright, P.L., 28870 U.S. Hwy. 19 N., 3rd Floor, Clearwater, FL 33761. Notices of claim must include: 1) the name and address of the claimant, and 2) the nature and amount of the claim. Pursuant to Florida Statute 607.1407, any claim against the corporation will be barred unless a proceeding to enforce the claim is commenced within 4 years after the date of the second consecutive weekly publication of this notice.
