You are HEREBY notified that a Petition for Termination of Parental Rights has been filed for Kendra Glenn a/k/a Baby Girl Corbett, a female child born on February 15, 2011 to Natalie Heather Braynen a/k/a Natalie Heather Corbett. The child was born at St. Joseph's Women's Hospital in Tampa, Hillsborough County, Florida. Nathaniel Norris Braynen is a black male, age 40, 5'8” tall, 200 pounds with brown eyes and black hair.
A Petition for Termination of your Parental Rights has been filed and you are required to serve a copy of your written answer to Gift of Life Adoptions, 4437 Park Blvd., Pinellas Park, Florida 33781, (727)549-1416, on or before June 30, 2011 at 9 a.m. and file the original with the Clerk of Court at the Sixth Judicial Circuit, Family Division - Unified Family Court, located at 501 - 1st Ave. north, St. Petersburg, Florida 33701 or 315 Court Street, Clearwater, Florida 33756. You are also required to appear at the hearing on the Petition for Termination of Parental Rights on June 30, 2011 at 9 a.m. in courtroom #13 at 14250 - 49th Street North, Clearwater, Florida 33762.
If you fail to file a written answer and appear at the hearing, a default may be entered against you and the Court may enter an order terminating any parental rights you may have concerned this child. Copies of all court documents in this case, including orders, are available to the Clerk of the Circuit Court's office. You may review these documents upon Court Order.
You must keep the Clerk of the Circuit Court's Office notified of your current address. (You may file Notice of Current Address, Florida Family Law Form 12.915). Future papers in this lawsuit will be mailed to the address on record at the Clerk's office.
