Plaintiff's attorney, whose address is 2600 North Military Trail, 4th Floor, Boca Raton, Florida 33431-6348, within 30 days after the date of the first publication of this notice, and file the original with the Clerk of this Court, otherwise a default will be entered against you for the relief demanded in the complaint or petition.
