An ADMINISTRATIVE COMPLAINT to suspend or revoke your license (s) and eligibility for licensure and appointment has been filed against you. You have the right to request a hearing pursuant to Sections 120.569 and 120.57 (1) and (2), Florida Statutes, by mailing a request for same to Julie Jones, DFS Agency Clerk at the Department of Financial Services, Division of Legal Services, 200 E. Gaines St., Tallahassee, Florida 32399-0333. If a request for hearing is not received by December 9, 2011, the right to a hearing in this matter will be waived and the Chief Financial Officer will dispose of this case in accordance with the law.
