Notice is hereby given that Bay Injury & Rehab, Inc., a Florida corporation, with its principal place of business located at 4800 Fourth Street North, St. Petersburg, Pinellas County, Florida desires to engage in business under the fictitious name of Back & Neck Pain Clinic and intends to register that name with the Florida Department of State, Division of Corporations, pursuant to Florida Statutes, Section 865.09, and to engage in business under that name.
