G., a male child born on the 10th day of March, 2006, in Pinellas County, Florida, for subsequent adoption and you are hereby commanded to appear on May 8, 2012, at 2:00 p.m. before the Honorable Lynn Tepper, Judge of the above-styled Court at the Robert D. Sumner Judicial Center, 38053 Live Oak Avenue, Dade City, FL 33523.
YOU MUST APPEAR ON THE DATE AND AT THE TIME SPECIFIED. YOUR FAILURE TO APPEAR WILL BE TREATED AS A CONSENT TO TERMINATION OF PARENTAL RIGHTS AND YOU SHALL LOSE ALL LEGAL RIGHTS AS A PARENT TO THE CHILD NAMED IN THE PETITION FOR TERMINATION OF PARENTAL RIGHTS.
