YOU WILL TAKE NOTICE that you have been sued in the above-named Court by the above-named Petitioner, to Determine Custody; Establish Child Support and Motion to Change Name, and that unless you answer the Petition filed by this Petitioner in said Court on or before the 12 day of July, 2012, said Petition will be taken as true and judgment granting to the Petitioner for damages rendered according to the prayer thereof. Notice is also given that the Court has set a hearing on the Petition on the 12 day of July, 2012, in Courtroom No. 1 at 9:00 a.m.
