REQUEST FOR PROPOSALS
Construction Services for
Phase 3 of Forest Creek Residential Subdivision Infrastructure
Manatee County, Florida
Forest Creek Community Development District (the “District”) hereby requests proposals to provide services relating to the construction and/or installation of various improvements within the Phase 3 of Forest Creek Residential Subdivision near 11645 Old Florida Lane, Parrish, FL 34219 including, but not limited to, earthwork, storm drains, water main, sewer main and irrigation mains as more specifically set forth in the Project Manual.
The Project Manual will be available beginning Monday, May 14, 2012, at 12:00 p.m. from Neal Communities, 8210 Lakewood Ranch Boulevard, Lakewood Ranch, Florida, 34202. Each Project Manual will include, but not be limited to, the Request for Proposals, proposal and contract documents, and construction plans and specifications. Ms. Pam White, District Accountant, shall be the contact person with regard to the Project Manual. Ms. White can be reached at [email protected] or 941-328-1091.The District has the right to approve a proposal or contract, reject any and all proposals, make modifications to the work, and waive any minor informalities and irregularities in proposals as it deems appropriate, if it determines in its discretion that it is in the best interest of the District to do so.
Any person who wishes to protest the Project Manual, or any component thereof, shall file with the District a written notice of protest within seventy-two (72) hours after the Project Manual is made available, and shall file a formal written protest with the District within seven (7) calendar days after the date of timely filing the initial notice of protest. Filing will be perfected and deemed to have occurred upon receipt by the District Manager, Development Planning & Financing Group, Inc., 15310 Amberly Drive, Suite 175, Tampa, Florida 33647. Failure to timely file a notice of protest or failure to timely file a formal written protest shall constitute a waiver of any right to object to or protest the contents of the District's Project Manual. The formal written protest shall state with particularity the facts and law upon which the protest is based.
Ranking of proposals will be made on the basis of qualifications according to the evaluation criteria contained within the Project Manual. Any and all questions relative to this project shall be directed in writing only to Ms. Pam White, Neal Communities 8210 Lakewood Ranch Boulevard, Lakewood Ranch, Florida, 34202, via facsimile 941-328-1100 or e-mail at [email protected], no later than Tuesday, May 22, 2012 at 4:00 p.m.
Firms desiring to provide services for this project must submit one (1) original and eight (8) copies of the required proposal no later than 1:00 p.m. on Friday, June 22, 2012, at the office of Neal Communities, 8210 Lakewood Ranch Boulevard, Lakewood Ranch, Florida 34202, Attention: Ms. Pam White “Phase 3 RFP”. Proposals shall be submitted in a sealed opaque package, shall bear the name of the proposer on the outside of the package and shall identify the name of the project. Proposals will be opened at the time and date stipulated above; those received after the time and date stipulated above will be returned un-opened to the proposer. Any proposal not completed as specified or missing the required proposal documents as provided in the Project Manual may be disqualified.
