YOU ARE NOTIFIED that an action for Termination of Biological Father's Parental Rights and Step-Parent Adoption has been filed. The minor subject to this petition was born on February 27, 2008 in Orlando, FL. There will be a hearing on the petition to terminate parental rights pending adoption on December 18, 2012 at 9:30AM before Judge Robert Evans at Orange County Courthouse, 425 N. Orange Ave., Orlando, FL 32801, Courtroom 16-F. The court has set aside 30 min. for this hearing. UNDER SECTION 63.089, FLORIDA STATUTES, FAILURE TO TIMELY FILE A WRITTEN RESPONSE TO THIS NOTICE AND THE PETITION WITH THE COURT AND TO APPEAR AT THIS HEARING CONSTITUTES GROUNDS UPON WHICH THE COURT SHALL END ANY PARENTAL RIGHTS YOU MAY HAVE OR ASSERT REGARDING THE MINOR CHILD.
You are required to serve a copy of your written defenses, if any, on Jamie Grosshans, Attorney for Nhan Do and Morgan Do, at 100 W. Plant. St., Winter Garden, FL 34787 on or before the hearing date, and file the original with the clerk of this Court at 425 North Orange Avenue, Orlando, FL 32801 before service on Petitioner or immediately thereafter.

You must keep the Clerk of the Circuit Court's office notified of your current address. (You may file Notice of Current Address, Florida Supreme Court Approved Family Law Form 12.915). Future papers in this lawsuit will be mailed to the address on record at the clerk's office.
