DATED JULY 28, 1992, AS AMENDED AND RESTATED
1. Pauline V. Pollard, a resident of Collier County, Florida, who died on July 17 2012, was the settlor of a trust entitled the Pauline V. Pollard Revocable Trust dated July 28, 1992, as amended and restated, which is a trust described in Section 733.707(3) of the Florida Probate Code.
2. The name and address of the Trustee who has accepted such appointment is set forth below. The fiduciary lawyer-client privilege in Section 90.5021 F.S. applies with respect to the Trustee and any attorney employed by the Trustee.
3. You have the right upon reasonable request, to a complete copy to the trust instrument; accountings pursuant to Section 736.0813; relevant information about the assets and liabilities of the trust; and the particulars relating to administration.
4. Notice provided to the following Qualified Beneficiaries by certified mail this 1st day of February, 2013 and publication on February 8, 2013.
