Notice is hereby given that Florida Dermatology and Skin Cancer Specialists, PL, with its principal office located at 4200 North Armenia Avenue North, Suite 1, Tampa, Florida 33607 desires to engage in business under one or more of the following fictitious names: Armstrong Dermatology and Skin Cancer Center; Dermatology Specialists of West Florida, Forman Dermatology and Skin Cancer Institute; Florida Dermatology Specialists; and Florida Westcoast Skin and Cancer Center and intends to register those names with the Florida Department of State, Division of Corporations, pursuant to Florida Statutes, Section 865.09, and to engage in business under one or more of those names.
