COSENTINO; ROBERT M. EMBREY; MARIANNE EMBREY; NANCY A. COSENTINO; MORTGAGE ELECTRONIC REGISTRATION SYSTEMS INCORPORATED AS NOMINEE FOR JP MORGAN CHASE BANK N.A.
