You are hereby notified that a Petition under oath has been filed in the above-styled Court for the termination of your parental rights of Joseph Giannoccora, a male child, born on October 3, 2010 in Pinellas County, Florida to the mother, Megan Caver, and commitment of Joseph Giannoccora to the State of Florida Department of Children and Families for subsequent adoption. You are hereby noticed and commanded to be and appear before the Honorable Lorraine Kelly, Judge of the Circuit Court, at the Pinellas County Criminal Justice Center, 14250 49th Street North, Courtroom #13, Clearwater, Florida, 33762, on September 12, 2013 at 09:00 AM.
FAILURE TO PERSONALLY APPEAR AT THIS ADVISORY HEARING CONSTITUTES CONSENT TO THE TERMINATION OF PARENTAL RIGHTS OF Joseph Giannoccora. IF YOU FAIL TO PERSONALLY APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL YOUR LEGAL RIGHTS AS A PARENT TO Joseph Giannoccora NAMED IN THE PETITION. AN ATTORNEY CANNOT APPEAR FOR YOU.
