Zusman Eddy, as an Heir of the Estate of Jack Zusman, deceased, Deborah R. Wolfson a/k/a Deborah R. Zusman a/k/a Deborah R. Wolfson Zusman, as an Heir of the Estate of Jack Zusman, deceased, Ellen S. Zusman, as an Heir of the Estate of Jack Zusman, deceased, Ellen Zusman, as Co-Trustee under the provisions of a testamentary trust agreement dated January 9, 1998 known as The Jack Zusman Revocable Trust, Phyllis M.
