To: Donald Echelbarger, putative biological father of Mykala Jade Echelbarger, whose last address is Clearwater, Florida, and to any other unknown putative biological father:
You are hereby notified that on November 19, 2013, a Petition for Adoption of Mykala Jade Echelbarger, a female child born to Ida Mae Terry on December 9, 2001 in Birmingham, Alabama, was filed in the Superior Court of Cobb County, Georgia, Adoption No. 13A134-53.
All rights you may have with respect to the minor child will be lost and you will neither receive notice nor be entitled to object to the adoption of the child unless, within thirty (30) days from the date of the last publication of this notice, you: (1) file a petition to legitimate the child pursuant to Code Section 19-7-22; (2) file notice of the filing of the petition to legitimate or acknowledgment of legitimation with the court in which the adoption action is pending, and (3) provide notice to the undersigned attorney for the petitioner seeking to adopt the child.
