The State of Missouri to Isaac Guest, you are hereby notified that an action has been commenced in the Circuit Court for the County of Boone County, Missouri, the object and general nature of which is the Petition for Declaratory Judgment providing that you are not a qualified insured under Auto Club Family Insurance Company, Missouri Personal Auto Policy, Policy No. A5-512677-1.
The name and address of Plaintiff is Auto Club Family Insurance Company, P.O. Box 66502, St. Louis, Missouri 63166, and the name and address of the attorney for Plaintiff is Oliver Walker Wilson LLC, 401 Locust Street, Suite 406, P.O. Box 977, Columbia, Missouri 65205-0977. The names and addresses of the co-Defendants are Nina Guest, 2475 E. Highway 163, Columbia, MO 65201, and Glen Guest, 2475 E. Highway 163, Columbia, MO 65201.
You are further notified that, unless you file an answer or other pleading or shall otherwise appear and defend against the aforesaid Petition within 45 days after 12th day of December, 2014, the first date of publication, judgment by default will be rendered against you.
