NOTICE OF TERMINATION OF FLORIDA GUARDIANSHIP OF MINOR WARD AND CHANGE OF DOMICILE OF RESIDENT WARD
IN THE CIRCUIT COURT OF THE SIXTH JUDICIAL CIRCUIT IN AND FOR PINELLAS COUNTY, FLORIDA
PROBATE DIVISION

STANISLAW DEREWONKO,
Incapacitated.
The guardianship of STANISLAW DEREWONKO, (the Ward), is pending in the Circuit Court for Pinellas County, Florida, Probate Division; File No. 03-9432-GD-3, the address of which is 315 Court Street, Clearwater, FL 33756. The name and address of the Guardian and the Guardian's attorney are set forth below.
ALL INTERESTED PERSONS ARE NOTIFIED THAT:
1. The name of the Illinois Guardian is MARGARET PORTERA and her address is 114 N Third Ave, Des Plaines, IL 60016 and the name of the attorney for the Illinois Guardian is MARITESS BOTT and her address is 3701 Algonquin Road, Suite 712, Rolling Meadows, IL 60008.
2. A Petition for Discharge and Final Report and Accounting have been filed upon the grounds of change of domicile of the Ward.
3. The Florida Guardian will apply for discharge on or after January 26, 2015.
4. Upon entry of the Order of Discharge, the jurisdiction of the Ward will be transferred to Cook County, Illinois.
5. You are required to file with the Clerk of the above Court within the later of thirty (30) days from the date of service of the Petition for Discharge and Final Report and Accounting upon you or the date of first publication of this Notice, any objections to the Petition for Discharge and Final Report and Accounting and to serve a copy thereof on Petitioner's attorney, whose name and address is set forth below, and on all other interested persons including the attorney for the Illinois Guardian. Any objection must be in writing and must state with particularity each item to which the objection is directed and the grounds on which the objection is based. The objection is abandoned unless a Notice of Hearing is served on the Guardian, foreign Guardian, and other interested persons within ninety (90) days after the objection is filed.
All objections not so filed will be forever barred.
