YOU ARE NOTIFIED that a Petition to Determine Beneficiaries has been filed against you and you are required to serve a copy of your written defenses, if any, to it on Sherri M. Stinson, Petitioner's attorney, whose address is 569 S. Duncan Avenue, Clearwater, Florida 33756-6255, on or before 4-2-14, and file the original with the clerk of this court either before service on Petitioners' attorney or immediately thereafter; otherwise a default will be entered against you for the relief demanded in the complaint or petition.
