You are hereby commanded to appear before a Judge of the Circuit Court, Juvenile Division, in and for the County of Pinellas, and State of Florida, at the Criminal Justice Center, Courtroom 14, 14250 49th Street North, Clearwater, Florida 33762 on Thursday, April 17, 2014 at 9:00 a.m., for an Advisory Hearing on the Petition for Termination of Parental Rights.
FAILURE TO PERSONALLY APPEAR AT THIS HEARING CONSTITUTES CONSENT TO THE TERMINATION OF YOUR PARENTAL RIGHTS OF THESE CHILDREN. IF YOU FAIL TO APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL LEGAL RIGHTS AS A PARENT TO THE CHILDREN NAMED IN THE PETITION.
YOU HAVE THE RIGHT TO BE REPRESENTED BY A LAWYER. IF YOU CANNOT AFFORD ONE, THE COURT WILL APPOINT ONE FOR YOU. YOU ARE CURRENTLY REPRESENTED BY THE OFFICE OF THE REGIONAL CONFLICT COUNSEL.
