In accordance with the requirements of Florida Statues, Section 736.05055, the below named Trustee files this Notice of Trust for Settlor, Lorraine E. Fricek, and says:
1. Lorraine E. Fricek, Settlor of the Lorraine E. Fricek Revocable Trust executed by her on May 1, 2007, hereinafter the Trust.
2. Lorraine E. Fricek was authorized to amend or revoke the Trust.
3. Lorraine E. Fricek died on September 23, 2014 and at the time of her death her residence and domicile was at 1255 S. Pasadena Avenue, St. Petersburg, Pinellas County, Florida 33707.
4. As of the date of death of Settlor, Lorraine E. Fricek did have the right of revocation of the Trust.
5. The name and address of the Successor Trustee of the Trust, now serving as Trustee, are: Terryl Jean Stirling, 1809 Tanglewood Drive, Unit J, Glenview, IL 60025, Pinellas County, Florida.
