In accordance with the requirements of Florida Statues, Section 736.05055, the below named Trustee files this Notice of Trust for Settlor, Margaret K. Kulycky, and says:
1. Margaret K. Kulycky was Settlor of the Margaret K. Kulycky Revocable Trust executed by her on June 6, 2012, hereinafter the Trust.
2. Margaret K. Kulycky died on May 29, 2014 and at the time of her death her residence and domicile was at 8333 Seminole Blvd., Unit #247E, Seminole, Pinellas County, Florida 33772.
3. As of the date of death of Settlor, Margaret K. Kulycky did have the right of revocation of the Trust.
4. The name and address of the Successor Trustee of the Trust, now serving as Trustee, are: Martha Hordinsky, P.O. Box 1144, Oklahoma City, OK 73101-1144.
