IN RE: THE TRUST AGREEMENT OF TALMAGE L. CAMP TRUST DATED MAY 27, 2005,
AMENDED APRIL 13, 2009 AND APRIL 22, 2009

TO ALL PERSONS HAVING CLAIMS OR DEMANDS AGAINST THE ABOVE TRUST:
You are hereby notified that Talmage L. Camp, the Grantor of the Trust Agreement of Talmage L. Camp, dated May 27, 2005, Amended on April 13, 2009 and April 22, 2009, died on April 8, 2014, a resident of Hillsborough County, Florida. The name and address of the Trustee serving at the time or as the result of the Grantor's death, and the attorney for the Trustee, is set forth below.
ALL INTERESTED PERSONS ARE NOTIFIED THAT:
All creditors of the decedent or his trust and other persons having claims or demands against the decedent or his trust on whom a copy of this Notice is served within three (3) months after the date of the first publication of this Notice, must file their claims with the Trustee, whose name appears below, WITHIN THE LATER OF THREE (3) MONTHS AFTER THE DATE OF THE FIRST PUBLICATION OF THIS NOTICE OF THIRTY (30) DAYS AFTER THE DATE OF SERVICE OF A COPY OF THIS NOTICE ON THEM.
All other creditors of the decedent and persons having claims or demands against the decedent or his trust, must file their claims with the Trustee, whose names appear below, by any form of mail requiring a signed receipt, WITHIN THREE (3) MONTHS AFTER THE DATE OF THE FIRST PUBLICATION OF THIS NOTICE. ALL CLAIMS AND DEMANDS NOT SO FILED WILL BE FOREVER BARRED.
NOTWITHSTANDING THE TIME PERIODS SET FORTH ABOVE, ANY CLAIM FILED (TWO) YEARS OR MORE AFTER THE DECEDENT'S DATE OF DEATH IS BARRED.
