Banks, deceased, File Number 15-3267-ES-4, by the Circuit Court for Pinellas County, Florida, Probate Division, the address of which is 315 Court Street, Clearwater, Florida 33756; that the decedent's date of death was February 17, 2015; that the total value of the estate is $8,924.79 and that the names and addresses of those to whom it has been assigned by such order are:
