YOU ARE NOTIFIED that an action for dissolution of marriage has been filed against you and that you are required to serve a copy of your written defenses, if any, to it on James D. Beach, Esquire, Attorney for Petitioner, whose address is James D. Beach, P.A., 100 Second Avenue North, Suite 240, St. Petersburg, FL 33701 on or before 6-26-2015, and file the original with the Clerk of this Court at 315 Court Street, Clearwater, FL 33756 before service on Petitioner's attorney or immediately thereafter. If you fail to do so, a default may be entered against you for the relief demanded in the Petition.
The action is asking the court to decide how the following real and personal property should be divided:

The 401 (k) of Hue Le Miller
The mortgage note held by both parties on the previously jointly owned real property located at 3629 Wellington Drive, Holiday, Pasco County, Florida 34691, which sold for the sum of Sixty Thousand Dollars ($60,000.00) on May 15, 2013. The balance is schedule to be paid off at the rate of Five Hundred Ninety-One Dollars and Thirty-Seven Cents ($591.37) per month from July 1, 2013 to June 1, 2021.
Copies of all court documents in this case, including orders, are available at the Clerk of the Circuit Court's office. You may review these documents upon request.
You must keep the Clerk of the Circuit Court's office notified of your current address. (You may file Notice of Current Address, Florida Supreme court Approved Family Law Form 12.915.) Future papers in this lawsuit will be mailed to the address on record at the clerk's office.
