You are hereby notified that a Petition for Termination of Parental Rights pending Stepparent Adoption has been filed in the Circuit Court of the Sixth Judicial Circuit of Florida, In and for Pinellas County for the children known as SMC born June 16, 2003, in Phoenix, Arizona, P.M.C. born February 25, 2006, in Phoenix, Arizona and M.R.C. born February 20, 2008 in Clearwater, Florida to Melissa Holmes, f/k/a Melissa Cartwright. This Petition requests the Court to waive your consent to the stepparent adoption under a claim of abandonment. You are hereby notified that you must file a written response to the Termination of Parental Rights for the adoption of the minor children with the Clerk of the Circuit Court, located in the Pinellas County Courthouse, 315 Court Street, Clearwater, Florida 33756. Your written response, including the case number given above and the names of the parties, must be filed if you want the Court to hear your side of the case on or before DEC. 18, 2015
If you do not file your written response on time, you may lose the case, and any parental rights you may have regarding the minor children may be terminated thereafter without further warning from the Court. There are other legal requirements. You may want to call an attorney right away. If you do not know an attorney, you may call an attorney referral service or legal aid office (listed in the phone book), or you may be entitled to have the Court appoint legal counsel to represent you in this action.
