VIRGINIA OLGA LEE, a resident of Sarasota County, Florida, who died on December 24, 2015, was the Grantor of a Trust entitled VIRGINIA OLGA LEE REVOCABLE TRUST AGREEMENT dated March 22, 2006, and as amended on August 8, 2006, March 12, 2007, May 22, 2007, June 4, 2007, August 5, 2008, September 2. 2008, May 6, 2009, September 3, 2010, February 15, 2012. June 7, 2012, December 7, 2013, December 19,20 13, April 1, 2014, and September 23,2014 (the “Trust”). Under Florida Statute 736.0604, an action to contest the validity of a Trust that was revocable at the time of the Settlor's death is barred if not commenced within the earlier of: (1) the time as provided in Chapter 95 of the Florida Statutes: or (2) six months after the Trustee sent the person a copy of the Trust Instrument and a Notice informing the person of the Trust's existence, of the Trustee' s name and address, and of the time allowed for commencing a proceeding.
Accordingly, we are providing you with the following information:
1. Notice that the VIRGINIA OLGA LEE REVOCABLE TRUST AGREEMENT dated March 22, 2006, and as amended on August 8, 2006, March 12, 2007, May 22, 2007, June 4, 2007, August 5, 2008, September 2, 2008. May 6, 2009, September 3, 2010. February 15, 2012, June 7, 2012, December 7, 2013, December 19, 2013, April 1, 2014, and September 23, 2014) existed at the death of VIRGINIA OLGA LEE who died on December 24, 20 14;
2. A copy of the Trust Agreement is supplied herewith;
3. The Trustee's name and address is:
Jonene Eisch, P.O. Box 1490, Bradenton, FL 34206-1490.
4. Under Florida Statute 736.0604, the time allowed for commencing a proceeding is six (6) months after the date of this Notice is sent to you, as set forth below.
