YOU ARE NOTIFIED that a Petition for Change of Name of Minor Child has been filed against you, regarding the following minor child: Aleiyah Gage Magnuson, born March 17, 2012.
You are required to serve a copy of your written defenses, if any, to Petitioner's attorney, Maureen C. Chiofalo, Esquire, whose address is: 1001 3rd Avenue W., Suite 650, Bradenton, FL 34205, on or before May 9, 2016 and file the original with the clerk of this court either before service on Petitoner's attorney or immediately thereafter. If you fail to do so, a default will be entered against you for the relief demanded in the petition.
Copies of all court documents in this case, including orders, are available at the Clerk of the Circuit Court's office. You may review these documents upon request.
