YOU ARE NOTIFIED that a Notice of Intended Adoption Plan which may result in the termination of your parental rights regarding a child born on February 21, 2016, at Morton Plant Hospital in Clearwater, Florida, has been filed against you, and you are required to serve a copy of your written defenses, if any, to it on ROBERT J. KELLY, ESQUIRE, Attorney for the Petitioner, whose address is 605 Palm Blvd. Suite A, Dunedin, FL 34698, on or before 4-1-, 2016, and file the original with the clerk of this court either before service on the Attorney for the Petitioner, or immediately thereafter. If you fail to do so, a default may be entered against you for the relief demanded in the Notice and any subsequent petition for termination of parental rights.
Copies of all court documents in this case, including orders, are available at the Clerk of the Circuit Court's office. You may review these documents upon request.
