STATE OF FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION NOTICE OF ISSUANCE OF SITE REHABILITATION COMPLETION ORDER
The Florida Department of Environmental Protection (FDEP) gives notice of the issuance of a Site Rehabilitation Completion Order with conditions (SRCO-C) for a contaminated site, FDEP Site #COM_286684, known as the former Countryside Executive Golf Course, 2506 Countryside Blvd., Clearwater, Pinellas County, FL (Property). The SRCO confirms that Countryside-Alexander Investments, LLC and 25 Countryside West, LLC has successfully and satisfactorily met the requirements of Chapter 62-780, FAC, site rehabilitation tasks related to activities and known releases originating on the Property and, accordingly, no further action is required with respect to such releases.

A person whose substantial interests are affected by FDEP's action may petition for an administrative proceeding (hearing) in accordance with Sections 120.569 and 120.57, FS. The petition must be received by the Agency Clerk, Department of Environmental Protection, 3900 Commonwealth Boulevard, Mail Station 35, Tallahassee, Florida 32399-3000, within 21 days of the publication of this notice.

The failure of any person to file a petition for an administrative hearing within the appropriate time period shall constitute a waiver of that person's right to request an administrative determination (hearing) under Sections 120.569 and 120.57, FS.
