THURNER A/K/A BETTY JEAN THURNER, REVOCABLE TRUST, UNDER DECLARATION DATED JULY 29, 1991, AS RESTATED IN ITS ENTIRETY ON JULY 26, 2001; UNKNOWN SUCCESSOR TRUSTEE OF THE BETTY J.
