YOU ARE NOTIFIED THAT the Petitioner, has instituted an action seeking to modify the Parenting Plan through the Supplemental Petition to Modify the Parenting Plan and Other Relief, a copy of the legal description of the same is attached hereto as Exhibit “A”.
You are required to serve a copy of your written defenses, if any, to this above-styled action on Maureen Avila, Esquire, of Avila Family Law Offices, Petitioner's attorney, whose address is 2075 Main Street., Ste. 20. Sarasota, Florida 34237; and on or before 6/27/16, and file the original with the Clerk of the Court either before service on the Petitioner's attorney or immediately thereafter; otherwise a default will be entered against you for the relief demanded in the Supplemental Petition to Modify the Parenting Plan and Other Relief.
