STATE OF FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION NOTICE OF PROPOSED AGENCY ACTION
The Florida Department of Environmental Protection (FDEP) gives notice that it proposes to approve a No Further Action Proposal with Conditions and issue a Site Rehabilitation Completion Order with Conditions for a contaminated site (Order). 25 Countryside West, LLC is seeking this Order in reference to the former Countryside Executive Golf Course, 2506 Countryside Boulevard, Clearwater, FL, FDEP ID Nos. COM_286684 and BF521401001, and intends to rely on a restriction on water use.

Complete copies of the No Further Action with Conditions proposal, existing institutional controls and FDEP's preliminary evaluation are available for public inspection during normal business hours 8:00 a.m. to 5:00 p.m., Monday through Friday, except legal holidays at the Florida Department of Environmental Protection, SW District, 13051 N. Telecom Pkwy., Temple Terrace, FL 33637-0926, attn: Bob Sellers.

Local governments with jurisdiction over the property subject to the institutional control, real property owner(s) of any property subject to the institutional control, and residents of any property subject to the institutional control have 30 days from publication of this notice to provide comments to FDEP. Such comments must be sent to Florida Department of Environmental Protection, SW District, 13051 N.
