Newlon, Esquire, whose address is 14141 5th Street, Dated City, FL 33525, on or before December 12, 2016, and file the original with the Hillsborough County Clerk of Court, 800 E. Twiggs Street, Tampa, FL 33602, before service on Petitioner, or immediately thereafter. If you fail to do so, a default may be entered against you for the relief demanded in the petition.
The action is for the name change of one (1) minor child, has been brought by the above-referenced Petitioner, and affects you as the legal father of the child.
Copies of all court documents in this case, including orders, are available at the Clerk of the Circuit Court's office. You may review these documents upon request.
