YOU ARE HEREBY NOTIFIED that a civil action has been filed against you in the Circuit Court of Polk County, Florida, to foreclose certain real property described as follows:
Lot 4, Block 1137, Poinciana, Neighborhood 5, Village 7, according to the plat thereof as recorded in Plat Book 53, Pages 19 through 28, inclusive, of the Public Records of Polk County, Florida.

You are required to file a written response with the Court and serve a copy of your written defenses, if any, to it on Timothy D.
