Petitioner/Mother, Yessica A. Beltran, being sworn, petitions this court for termination of parental rights of the Father/Respondent of the minor child, and states as follows:
1. This is an action under Fla.Stat. 39.806(1). The grounds for the termination of parental rights may be established under any of the following circumstances. Section (b) abandonment as defined in s.39.01 (1) or when the identity or location of the parent or parents is unknown and cannot be ascertained by diligent search within 60 days.
2. The minor child (ren) subject to this petition is/are:
William Alejandro Castrejon-Beltran
D.O.B.: November 6, 2008
Naples, Florida
A certified copy of the birth certificate of the minor child attached.
3. The minor child has not used any other names.
4. A completed Uniform Child Custody Jurisdiction Affidavit (UCCJA), Florida Supreme Court Approved Family Law Form 12.902(d), is filed with this petition.
5. The grounds for terminating parental rights are:
a. Father/Respondent left the child when he was a year old.
b. Father/Respondent has not had any type of contact with the Petitioner/Mother or the child.
6. The Respondent/Father does not reside in the United States.
7. The Respondent/Father has not had any contact with the child since the child was 4 months old. (Please see attached affidavit).
8. It is in the child's best interest to remain with the Petitioner/Mother.
WHEREFORE, I request that this Court enter a Final Judgment of Termination of Parental Rights Pending for the Respondent in the above styled matter.
