You are hereby notified that a Petition under oath has been filed in the above-styled Court for the termination of your parental rights of A.M., a male child, born on July 30, 2015, in Pinellas County, Florida, to the mother, Amber Buckner, and commitment of this child to the State of Florida Department of Children and Families for subsequent adoption. You are hereby noticed and commanded to be and appear before the Honorable Kimberly Todd, Judge of the Circuit Court, at the Pinellas County Justice Center, 14250 49th Street North, Courtroom 14, Clearwater, Pinellas County, Florida, 33762, on May 16, 2017 at 10:00 a.m.
FAILURE TO PERSONALLY APPEAR AT THIS ADVISORY AND ADJUDICATION/DISPOSITION HEARING CONSTITUTES CONSENT TO THE TERMINATION OF PARENTAL RIGHTS OF THIS CHILD. IF YOU FAIL TO PERSONALLY APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL YOUR LEGAL RIGHTS AS A PARENT TO THIS CHILD NAMED IN THE PETITION. AN ATTORNEY CANNOT APPEAR FOR YOU.
