You are hereby notified that a Petition under oath has been filed in the above-styled Court for the termination of your parental rights of A.W., a male child, born on November 28, 2014 in Sebring, Highlands County, Florida, and K.W., a female child, born on October 11, 2013 in Sebring, Highlands County, Florida, to the mother, Erica Wooden, and commitment of these children to the State of Florida Department of Children and Families for subsequent adoption. You are hereby noticed and commanded to be and appear before the Honorable Patrice Moore, Judge of the Circuit Court, at the Pinellas County Justice Center, 14250 49th Street North, Courtroom 14, Clearwater, Pinellas County, Florida, 33762, on Monday, April 3, 2017 at 9:00 a.m.
FAILURE TO PERSONALLY APPEAR AT THIS ADVISORY HEARING CONSTITUTES CONSENT TO THE TERMINATION OF PARENTAL RIGHTS OF THESE CHILDREN. IF YOU FAIL TO PERSONALLY APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL YOUR LEGAL RIGHTS AS A PARENT TO THESE CHILDREN NAMED IN THE PETITION. AN ATTORNEY CANNOT APPEAR FOR YOU.
