Smoak by the Circuit Court, Pinellas County, Florida, Probate Division, the address of which is: PINELLAS COUNTY COURTHOUSE, 315 Court Street, Clearwater, Florida 33756; that the decedent's date of death was June 24, 2016; that the total value of the estate was under $1,000.00 and that the names and address of those to whom it has been assigned such order are:
