Howard, YOU ARE HEREBY NOTIFIED that an action for Declaratory Judgment on the LOIS M. HOWARD REVOCABLE TRUSTE U/T/D October 1, 1990, has been filed against you and you are required to serve a copy of your written defenses, if any, to plaintiff's attorney, whose name and address is:
