STATE OF FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION NOTICE OF ISSUANCE OF SITE REHABILITATION COMPLETION ORDER
The Florida Department of Environmental Protection (FDEP) gives notice of the issuance of a Site Rehabilitation Completion Order with conditions (SRCO-C) for a
contaminated site, FDEP ID# COM_75928, known as the former Shields Marine and Concept Inc. Facility, 12707 US Highway 19 North, Clearwater, Pinellas County
(Property). The SRCO confirms that MetLife Real Estate has successfully and satisfactorily met the requirements of Chapter 62-780, FAC, site rehabilitation tasks related to known releases originating on the Property and, accordingly, no further action is required with respect to such releases.

The files associated with this order are available for public inspection during normal
business hours 8:00 a.m. to 5:00 p.m., Monday through Friday, except legal holidays at FDEP, SW District, 13051 N. Telecom Pkwy., Temple Terrace, FL 33637-0926, attn: Simone Core, or online at http://depedms.dep.state.fl.us/Oculus/servlet/login, Facility identification # COM_75928.

A person whose substantial interests are affected by the Department's action may petition for an administrative proceeding (hearing) in accordance with Sections 120.569 and 120.57, FS. The petition must be received by the Agency Clerk, Department of Environmental Protection, 3900 Commonwealth Boulevard, Mail Station 35, Tallahassee, Florida 32399-3000, within 21 days of the publication of this notice.

The failure of any person to file a petition for an administrative hearing within the appropriate time period shall constitute a waiver of that person's right to request an
administrative determination (hearing) under Sections 120.569 and 120.57, FS.
