You are hereby notified that a Petition for Termination of Parental Rights has been filed in the Circuit Court of the Sixth Judicial Circuit, In and For Pinellas County, Florida for the child currently known as B.G.T., born March 27, 2019 in Pinellas County, Florida. This Petition requests the Court to waive your consent to adoption under a claim of abandonment. You are hereby notified that you must file a written response to the Petition for Termination of Parental Rights, with the Clerk of the Circuit Court, located at Pinellas County Courthouse, 315 Court Street, Clearwater, Florida 33756. Your written response, including the case number given above and the names of the parties, must be filed if you want the Court to hear your side of the case on or before 9-6-, 2019.
If you do not file your written response on time, you may lose the case, and any parental rights you may have regarding the minor child may be terminated thereafter without further warning from the Court. There are other legal requirements. You may want to call an attorney right away. If you do not know an attorney, you may call an attorney referral service or a legal aid office (listed in the phone book), or you may be entitled to have the Court appoint legal counsel to represent you in this action. If you choose to file a written response yourself, at the same time you file your written response to the Court, you must also mail or take a copy of your written response to:

Additionally, you are hereby commanded to appear at a FINAL HEARING ON PETITION FOR TERMINATION OF PARENTAL RIGHTS, on September 11, 2019 at 9:00 a.m. before the Honorable Patrice Moore at the Pinellas County Justice Center, 14250 49th St. N., Clearwater, FL 33762, in Courtroom #13.
WARNING: UNDER SECTION 63.089, FLORIDA STATUTES, FAILURE TO TIMELY FILE A WRITTEN RESPONSE TO THIS NOTICE AND THE PETITION WITH THE COURT AND TO APPEAR AT THIS HEARING CONSTITUTES GROUNDS UPON WHICH THE COURT SHALL END ANY PARENTAL RIGHTS YOU MAY HAVE OR ASSERT REGARDING THE MINOR CHILD.
