This Notice hereby advises patients of his death, and further advises patients that they may obtain copies of their dental records at the following address: 127 North Garden Avenue, Clearwater, FL 33775. Additionally, Raymond G. Behm, Jr., P.A. (doing business as Behm Natural Dentistry) has sold the dental practice thereunder to Beata A Carlson 2, LLC, a Florida limited liability company, which LLC will continue to hold the records and files of all patients of Dr. Behm's practice at the address listed above.
