RESTATED AS OF JUNE 10, 2014
TO ALL PERSONS HAVING CLAIMS OR DEMANDS AGAINST THE ABOVE TRUST:
You are hereby notified that C. DAVID CARLEY, JR., the Surviving Grantor of The Carley Family Revocable Trust dated December 9, 1998, Restated as of June 10, 2014, died on August 21, 2019, a resident of Pinellas County, Florida. The name and address of the Co-Trustees serving at the time or as the result of the Grantor's death, and the attorney for the Co-Trustees, are set forth below.
ALL INTERESTED PERSONS ARE NOTIFIED THAT:
All creditors of the decedent or his trust and other persons having claims or demands against the decedent or his trust on whom a copy of this Notice is served within three (3) months after the date of the first publication of this Notice, must file their claims with the Co-Trustees, whose names appear below, WITHIN THE LATER OF THREE (3) MONTHS AFTER THE DATE OF THE FIRST PUBLICATION OF THIS NOTICE OR THIRTY (30) DAYS AFTER THE DATE OF SERVICE OF A COPY OF THIS NOTICE ON THEM.
All other creditors of the decedent and persons having claims or demands against the decedent or his trust, must file their claims with the Co-Trustees, whose names appear below, by any form of mail requiring a signed receipt, WITHIN THREE (3) MONTHS AFTER THE DATE OF THE FIRST PUBLICATION OF THIS NOTICE.
ALL CLAIMS AND DEMANDS NOT SO FILED WILL BE FOREVER BARRED.
NOTWITHSTANDING THE TIME PERIODS SET FORTH ABOVE, ANY CLAIM FILED TWO (2) YEARS OR MORE AFTER THE DECEDENT'S DATE OF DEATH IS BARRED.
