YOU ARE HEREBY NOTIFIED that an action to foreclose on a Claim of Lien has been filed against you on the following real property, lying and being and situated in POLK County, FLORIDA more particularly described as follows:
Lot 40D, HAMPTON HILLS SOUTH PHASE 3, a subdivision according to the plat thereof recorded at Plat Book 133, Pages 36 through 39, inclusive, in the Public Records of Polk County, Florida.
