The administration of the estate of JUDITH KAYE KOLISH, deceased, Case No: 21-CP-000840 is pending in the Circuit Court for Polk County, Florida, Probate Division, the address of which is 225 N. Broadway Ave., Bartow, Florida, 33830. The names and addresses of the Petitioner and the Petitioner's attorney are set forth below.
