You are hereby notified that a Summons, Verified Joint Petition for Adoption of a Minor Child, UCCJEA Affidavit, and Uniform Child Custody Jurisdiction and Enforcement Act Affidavit, has been filed regarding, Adalynn-Sonia Melendez, d/o/b 02/01/2015, in this Court on October 14, 2021. You are required to serve a copy of your written defenses to it, if any, on the Petitioner's attorney, whose name and address is: LORI W. CLIFFORD, ESQ., CLIFFORD FAMILY LAW, P.A., PO Box 9207, Fort Myers, FL 33902, and file an original with the clerk of this court on or before Jan 10, 2022 otherwise a Judgment may be entered against you for the relief demanded in the Petition.
Copies of all court documents in this case, including orders, and Notices of Hearing, are available at the Clerk of the Circuit Court's office. You may review these documents upon request.
You must keep the Clerk of the Circuit Court's office notified of your current address. (You may file Designation of Current Mailing and E-Mail Address, Florida Supreme Court Approved Family Law Form 12.915) Future papers in this lawsuit will be mailed or e-mailed to the addresses on record at the clerk's office.
UNDER SECTION 63.089, FLORIDA STATUTES, FAILURE TO FILE A WRITTEN RESPONSE TO THIS NOTICE AND THE PETITION WITH THE COURT AND TO APPEAR AT THIS HEARING CONSTITUTES GROUNDS UPON WHICH THE COURT SHALL END ANY PARENTAL RIGHTS YOU MAY HAVE OR ASSERT REGARDING THE MINOR CHILD.
