1. PLEASE TAKE NOTICE that on or about July 19, 2023, petitions were filed commencing Assignment for the Benefit of Creditors cases, pursuant to Chapter 727, Fla. Stat., made by A-1 Imaging Centers, LLC, Ambulatory Services Corporation, and Consolidated Healthcare Services, LLC, Assignors, with their principal place of business at 1800 2nd St, Ste 740, Sarasota, FL 34236, to Larry S. Hyman, Assignee, whose address is PO Box 18625, Tampa, FL 33679. The Petition was filed in the Circuit Court of Sarasota County.
2. YOU ARE HEREBY FURTHER NOTICED that pursuant to Fla. Stat. ยง727.105, no proceeding may be commenced against the Assignees except as provided in Chapter 727, and excepting the case of the secured creditor enforcing its rights in collateral under Chapter 679, there shall be no levy, execution, attachment or the like, in connection with any judgment or claim against assets of the Estates, other than real property, in the possession, custody or control of the Assignees.
3. YOU ARE HEREBY FURTHER NOTIFIED that in order to receive any dividend in this proceeding, you must file a Proof of Claim with the Assignee on or before November 16, 2023.
